Therapy Document Request

If you would like to request documents for therapy services you participated in at CYS, please follow the steps below. Prior to picking up your documents, total payment must be paid in full. Documents must be picked up in-person at our main CYS lobby during business hours. You will be required to provide proof of identification via photo ID. You will receive a treatment summary letter that include the information listed below.

 

Treatment Summary letter includes:
Referral Reason
Dates of Treatment
Diagnosis
Summary of Findings
Treatment Goals
Plan of Care (if applicable)
Current Status of Therapy

 

COST: $20 PER DOCUMENT REQUEST
(online payments are available: $21 (includes processing fees))
Allow two business days to complete requests
If needed sooner, an additional charge of $30 will be added
(online payment for additional charge is available: $31 (includes processing fees))

from Monday thru Friday from 8:00am – 5:00pm.

All online requests received during non-business hours of our CYS lobby will be processed the following business day.
